Here’s your PS 1583 “To-Do” List for New Mailbox Customers

1. Have Customer Complete PS Form 1583

  • Provide customer with PS Form 1583 (printed or digital).

    • You can enter their info into Bullship first, then download the Fillable 1583 PDF for them to fill out the rest

  • Ensure all fields are filled out completely and legibly.

2. Verify Their Identity

  • Collect two valid IDs:

    • Primary (with photo): Driver’s license, passport, etc.

    • Secondary (showing address): Utility bill, lease, etc.

  • Verify that both IDs are valid and match the customer’s information (ID cannot be a credit or debit card)

3. Witness Their Signature

  • Have the customer sign a printed and filled out PS Form 1583:

    • In-person (or virtually with real-time audio/video) in front of store staff

    • OR if remote, with a notarized signature.

4. Make Copies of IDs

  • Make clear, legible copies of both IDs (digital or paper copies)

  • Retain copies securely at the store (do not upload to USPS portal)

5. Upload the Form to the USPS Portal

  • Upload a scanned copy or photo of the completed and signed PS Form 1583 (no IDs) to the USPS CMRA Customer Registration Database.

6. Retain Those Records

  • File the signed PS Form 1583 + ID copies in your local store records.

  • Store securely (locked cabinet or encrypted digital storage).

  • Store records for:

    • Entire mailbox rental duration

    • PLUS 6 months after the mailbox is closed

7. Keep Records Available for Inspection

  • Ensure records are easily accessible to USPS officials (Postmaster, Postal Inspectors, the cops, etc.) upon request.
